An old iron amulet
From LSWiki
This is an ancient-looking piece of iron jewelry, composed of a necklace of dozens of small, round links connecting to a thin, coin-like disc. On one side of the disc is a relief of eight interlocking circles. On the other side is a rune that resembles a stylized key. Both sides are marred with nicks and scratches, and the reliefs are so faded in places that it is unclear if the complete representation remains. It is, despite being time- and travel-worn, perfectly-crafted; for all of its surface imperfections, it is exceptionally formed and remains oddly free of rust. It is permeated by a dim coruscating radiance. It has a few thin streamers of shimmering haze within and around it. It is in perfect condition. It is intermediate in size, and looks just about right to fit you. It appears to you as if it was crafted for a human. It looks about seven tenths of a dimin across and a tenth of a dimin long. It weighs about twenty-nine two-hundredths of a dekan. You estimate its value at about four thousand gold. You are wearing it.

After a moment, the knowledge that the old iron amulet is composed of fifty percent logos and fifty percent iron drifts into your mind like a long-forgotten memory.
A few moments after wearing:
You hear strange whispers of one who is called Yog-Sothoth, the Keeper of the Gate who is the Gate, the Keeper of the Key who is the Key.
After that:
You have a succession of strange, green-tinged visions of yourself near people and places visited long ago.
Visualizing yourself near a person or place:
You visualize yourself near the Grinning Cat Eatery of Camelot. Your old iron amulet feels momentarily cold, and you feel spiritually drained. You have a strangely vivid, green-tinged vision of yourself at the Grinning Cat Eatery of Camelot.
This amulet is normally carried by Yog-Sothoth, so it's quite rare to see it out in the world.
Through some secret mechanism (it is known, just that it's kept secret by its discoverers) the amulet can teleport you anywhere in the MUD. Yes, anywhere. Even, it's said, old and unlinked (unaccessible) places.
There's one known case of it teleporting an explorer into Oneiron and that person thereafter suffering a horrible death.
However, it eats your lives. A random amount at once. I do not know if it does this when teleporting, or while it's worn too. It can eat all of your lives at once, or at least an amount that's bigger than what you have.
As the above usage suggests it likely also can be used to teleport into any place you have memorized.
